Hello Lady Hawks Nation,
 
We are using Final Forms health database for the 2024-2025 school year. All information in the Final Forms database will need to be complete for your child's profile in order for them to try-out and participate on this year softball team. We are in the process of a coaching change at this time, please be patient with us.  Information about softball will come soon after school starts, have your child be listening for announcements. 
Your child's profile completion will be a simple process and will only take a few minutes to complete. All Final Forms information is below:

            -Proof of Residence (Please see Email Attachment for Form)

              -Current Utility Bill (Gas, Water, or Electric and must be less than 30 days old)

            -Physical Examination Form (Completed by Doctor during sports physical)

              -Medical Eligibility Form (Completed by Doctor during sports physical)
