If your Final Forms is not fully updated and cleared, you will not be permitted to participate in any workouts, tryouts, or practice.

-3 Residence Forms

            -Proof of Residence (Please see Email Attachment for Form)

              -Current Utility Bill (Gas, Water, or Electric and must be less than 30 days old)

              -Bus Zone Verification (Click this link for Zone Verification Website)

 

-2 Physical Forms

            -Physical Examination Form (Completed by Doctor during sports physical)

              -Medical Eligibility Form (Completed by Doctor during sports physical)

 

-Birth Certificate
